Well those days are over, enter the Micro Bluetooth Spy Earpiece. This teeny tiny device can be fitted deep inside your ear so no one will be bale to see it thus making it so much easier to remain undetected. It's designed to sync with almost all Bluetooth enabled handsets and although it's inside your ear, still provide clear reception of what's going on in the surroundings as well as communicate with the wearer. A magnet is used to extract the device from the users ear canal, seems like the only possible way to do so.

The earpiece also comes with a neck loop transmitter, microphone and a Morse Code button for extreme emergencies. Of course that means you'll have to learn Morse Code. The button, attached to a long wire, can be placed anywhere on the person so as to remain undetectable to anyone else. While I believe technology is slowly getting smaller and smaller for easier usage and yes it's quite ingenious, I also think it seems like a bit of a security issue as it's virtually 'invisible' and being sold to the public.
